RN Case Management
Health Care Connection's RN Case Management services are dedicated to ensuring that the individual health care needs of our clients are met. Our Registered Nurses conduct in-depth assessments based on years of health care experience working with both elderly clients and adults with disabilities. Following the initial RN assessment an individually tailored health care program is implemented. Our goals are to improve the level of wellness and quality of life for our clients, and act as a local resource for families in and out of the area.
Our Registered Nurses provide the following services:
- Health and Wellness Assessment
- Vital signs: blood pressure, heart rate, temperature, respirations
- Nutrition/hydration evaluation
- Skin assessment and personal hygiene
- Home safety evaluation
- Schedule and assist with MD and therapy appointments
- Medication oversight
- Communicate with MD and pharmacist
- Monitor for side effects or drug interactions
- Organize medications for easy/safe daily use
- Obtain prescription refills as needed
- Home Assessments
- Assess safety of living environment
- Offer referrals for cleaning, grab bars, ramps
- Assess client's ability to live at home
- Recommend care facilities, adult day care, home health care
- Health Insurance Assessments
RN Case Management services are offered at $75/hour.
